If you have an HMO plan or a plan that requires a referral, it is patient's responsibility to obtain the referral from your PCP before patient's visit. If we have not received the referral, we will be more than happy to see you as a private pay patient. All referrals must be made out to our primary physician, Dr. Lucy Li.

If your insurance plan contains a HRA (Health Reimbursement account), please provide a print out of your available funds at each visit.
